Step 1: Examine the contrast outline on the HSG. There is just one curved, elongated cavity, narrow and curved like a banana, ending in a single tube, and the whole structure sits to one side of the midline. That morphology is diagnostic.
Step 2: A single fusiform cavity with one tube means only one Mullerian duct formed properly - the definition of a unicornuate uterus, a class II Mullerian anomaly. It explains the off-midline position and the link with pregnancy loss and infertility.
Step 3: Contrast with the rivals. Didelphys would show two full cavities and two cervices, a bicornuate uterus would show two communicating horns, and a septate uterus would show a divided cavity within a normal outer contour. Only the lone banana-shaped laterally placed cavity matches the unicornuate type.
