Step 2: Identify it histologically - tumor cells encircle a true, virtually empty lumen, and thin cytoplasmic extensions reach inward toward that lumen, mimicking primitive photoreceptors. The same rosette can appear in other primitive neuroectodermal lesions like medulloblastoma and pineoblastoma.
Step 3: Contrast with Homer-Wright rosettes, where the centre is filled with fibrillary neuropil and which typify neuroblastoma and medulloblastoma. Wilms tumor and hepatoblastoma do not form this rosette, so they are excluded.
