Serovar approach. Chlamydia trachomatis is a single species split by serovar into three disease groups, and remembering the split answers this instantly. Serovars A to C cause trachoma (chronic conjunctivitis leading to blindness), serovars D to K cause genital chlamydia, neonatal conjunctivitis and pneumonia, and serovars L1 to L3 cause lymphogranuloma venereum. LGV runs in stages: a fleeting painless genital papule or ulcer, then tender inguinal and femoral lymphadenopathy that can show the pathognomonic groove sign, then late fibrosis. Knock out the wrong agents by their own classic STDs: Neisseria gonorrhoeae equals gonorrhoea, Treponema pallidum equals syphilis with a painless hard chancre, and Haemophilus ducreyi equals chancroid with a painful soft ulcer. Diagnosis is by nucleic acid amplification with L-serovar typing. Answer: Chlamydia trachomatis.
