Begin by grouping the options by mechanism. SLE, Grave's disease and myasthenia gravis all share one feature: the body attacks itself. In SLE the attack is broad and systemic through immune complexes deposited in skin, joints and kidneys. In Grave's disease a specific autoantibody mimics TSH and overdrives the thyroid. In myasthenia gravis antibodies block and destroy acetylcholine receptors, producing fatigable muscle weakness. Each of these three is therefore immunological in origin.
Sickle cell disease belongs to a completely different category. It is inherited, not acquired through immune dysregulation. A single nucleotide substitution converts glutamate to valine at position six of the beta chain, producing HbS that polymerises when deoxygenated and deforms red cells into sickles.
