Conceptual framework -- Form vs Content of Thought:
Psychiatry divides thought disorders along two axes: form (the way thoughts are linked and expressed) and content (what the thoughts are about).
Form of thought disorders include: derailment, thought blocking, tangentiality, circumstantiality, pressured speech, poverty of speech, verbigeration, perseveration, and flight of ideas. The unifying feature is a disruption in the logical flow and organisation of thought.
Content of thought disorders include: delusions (fixed false beliefs), obsessions (ego-dystonic repetitive thoughts recognised as one's own), thought insertion/withdrawal/broadcasting (passivity phenomena -- first-rank symptoms of schizophrenia), overvalued ideas, and phobias.
Applying to the options:
Option A -- Derailment: shift of thought from one track to another with no apparent connection -- this is a form disorder. Options B, C, D (Obsession, Thought insertion, Delusion) are all content disorders. Only derailment qualifies as a form of thought disorder.
