Step 1: Picture the microscope slide.
Small fluid filled spaces, each ringed by a neat circle of cells, so the whole thing looks like a miniature ovarian follicle trying to form. This ring and space unit is what pathologists call a Call-Exner body.
Step 2: Match the cell type that builds it.
Only granulosa cells organize themselves this way, producing the microfollicular pattern seen in granulosa cell tumor of the ovary. Finding these bodies on a slide is essentially a signature for this diagnosis.
Step 3: Walk through why the other three do not fit.
Theca cell tumor cells are plump and lipid rich but grow in solid sheets, they do not ring fluid spaces. Dysgerminoma is made of large cells with clear cytoplasm split by fibrous septa carrying lymphocytes, no rosettes at all. Brenner tumor shows nests of urothelial-type cells in dense fibrous stroma, again nothing like a follicle.
Step 4: State the answer.
