Investigation to localise site of haematuria =
CT (Computed Tomography)
Specifically:
CT IVP (CT Intravenous Pyelogram)
with three phases:
Arterial phase: detects active arterial extravasation
Venous/nephrographic phase: identifies parenchymal lacerations and extent of injury
Delayed/excretory phase: identifies collecting system disruption and ureteric injuries
AAST Renal Trauma Grading (based on CT):
Grade I: renal contusion or subcapsular haematoma
Grade II: non-expanding perirenal haematoma, laceration $<1$ cm depth
Grade III: laceration $>1$ cm without collecting system rupture
Grade IV: laceration extending into collecting system, vascular injury
Grade V: shattered kidney or avulsion of renal hilum
Why not others: X-ray cannot image parenchyma; USG misses collecting system injuries; MRI not practical in emergency.
