The task is to find the disease that is fairly evenly spread across the countryside and the cities, while the rest cluster more in one setting. Use exposure logic. Lung cancer rides on tobacco, traffic fumes and factory smoke, all of which are denser in towns, so it leans urban. Chronic bronchitis follows the same polluted air and smoking trail, again tilting urban. Mental illness shows real environmental patterning, with the pressures of city life shifting the spectrum of disorders, so it too differs by place.
Tuberculosis behaves differently. It thrives wherever there is crowding, undernutrition and poverty, and those conditions are not the exclusive property of either rural or urban India. As a result its burden is broadly shared and does not split neatly along the rural urban line, making it the odd one out.
