Step 1: Marfan's syndrome weakens elastic tissue through a fibrillin abnormality. The greatest mechanical and pulsatile stress in the arterial tree falls on the segment just above the aortic valve, so that is where weakening shows first.
Step 2: Histologically the wall shows cystic medial degeneration, meaning the structural elastin and smooth muscle are replaced by mucoid pools. With each heartbeat the aortic root and ascending aorta stretch and gradually balloon outward, producing an aneurysm and stretching the valve ring until the valve leaks.
Step 3: Compare the candidate sites. The arch, descending thoracic, and abdominal aorta endure less direct ventricular thrust and are involved much less commonly in Marfan's. The ascending aorta and root are the predictable site.
Step 4: The feared complications of this ascending aneurysm are aortic dissection and severe aortic regurgitation.
