Recall the five-part checklist for metabolic syndrome and the odd one out becomes obvious. The components are big waist (central obesity), high triglycerides, low HDL, high blood pressure and high fasting glucose, and you need any three to label the syndrome.
Run the options through that list: hypertriglyceridemia, central obesity and hypertension all sit squarely inside the criteria. The lipid abnormalities counted are specifically a high triglyceride level and a low HDL level. LDL cholesterol never appears in the NCEP-ATP III definition. So high LDL is the one that is not included, making option 1 correct. A handy way to remember it: metabolic syndrome cares about triglycerides up and HDL down, not LDL.
