The weakness here, drooping eyelids, a neck too weak to lift the head, and breathing difficulty, starts on day three, not on day one. Timing is the biggest clue in organophosphorus poisoning questions.

Low potassium can weaken muscles but is not the textbook cause here and does not explain ptosis. A Guillain-Barre type polyneuropathy usually follows an infection over a longer timeline, not a pesticide exposure. Polymyositis is a slow autoimmune disease, not something that appears three days after poisoning.
Step 5: Conclude.
The day three combination of ptosis, weak neck flexion and breathing difficulty after organophosphorus poisoning is the intermediate syndrome.
